· Most people say they would prefer to die at home, yet only about one-third of adults have an advance directive expressing their wishes for end-of-life care. Among those 60 and older, that number rises to about half of older adults completing a directive


· Having a living will is associated with decreased likelihood of dying in the hospital


· People without advance care planning are moved an average of three times in the last few weeks of life

Planning can provide a sense of relief and ease and decrease anxiety concerning end of life documents and wishes. Completing this workshop for yourself, your partner and your loved ones will mean that nobody is left wondering what your end of life wishes are and can prevent unwanted treatment and hospital visits. This is the best gift you can give your family.


Living Will- (AKA Advanced Directives) We utilize the Five Wishes Living Will. This easy to use Advanced Directives Booklet conveys your personal, emotional and spiritual needs as well as your medical wishes. It lets you choose your medical power of attorney (AKA Health Care Power of Attorney). It is used in the medical field to provide doctors and your family with your decisions about life-saving and life-prolonging treatments. The Five Wishes was written with the help of The American Bar Association’s Commission on Law and Aging and the nation’s leading experts in end of life care.


 Your Heart Will- While a traditional legally binding will bequeaths material goods, a heart will focuses on the non-material. A heart will is also known as an ethical will, a love will, spiritual will, legacy letters and ending notes. There are different formats for heart wills, they can be in the form of written letters, an audio recording, or a video. A heart will allows you to reveal old memories, personal stories, words of wisdom, life lessons, hopes, dreams and feelings. Heart wills can offer material goods as well, such as a favorite piece of jewelry to be gifted to a niece on a particular birthday. We can offer you support in deciding the content of your heart will as well as the format.


Inspired Endings Plan-The Inspired Endings Plan is your specific desires concerning the last days, months, or weeks of your life.  This is a map for your family, allowing them to know your specific desires such as who would you like at your bedside when you are dying, how you would like to be touched, specific readings such as poetry, spiritual or religious passages, meaningful rituals and more.


The combination of your living will, heart will and your Inspired Endings plan is a gift package for your loved ones. When you are finished they will know your wishes concerning your medical care and your spiritual and emotional needs at the end of life. They will know what was of value to you in your life and how that will be expressed in your final days.
